New Incoming Student Enrollment Information

Open Enrollment Notification:  
Frankfort-Elberta Area Schools open enrollment window for schools of choice will be from August 16 - August 30, 2017.  Enrollment will be open for all grades with the exception of grades first, second and third.  If parents have any questions about the open enrollment process, please call the school's Supertendent's Office at 231-352-4641.

-Schools of Choice Form:  If you live outside the FEAS district, but wish to attend a FEAS school, please complete the 2017-2018 Schools of Choice Form


 

Enrollment Forms
The following forms and requested information shall be completed and turned in prior to your child attending his/her first day at school with Frankfort-Elberta Area Schools:


 -Student Registration Form
 (for students new to the district)

 -Immunization record in accordance with state statute. Students who enroll in a public school who do not have up-to-date immunizations or an authorized waiver will not be allowed to attend school until appropriate paperwork is on file in the school office.  [view immunization requirements]

 -Certified birth certificate or other birth documentation if accompanied by a birth affidavit (A birth affidavit form can be obtained from any school office)

 -Special Education Form 
Additional information on how to fill the form out or for information about the enrollment process.

 -Proof of residency in the FEAS district.  An acceptable proof of residency may be a utility bill or lease which show a physical address.  A driver’s license will not be sufficed.



 

Additional Forms
The following forms are not required in order to attend FEAS, however, the information would allow FEAS to better serve you and your child:

 - Student Device Handbook and Insurance Contract - Student handbook for chromebooks and iPads.  All students must have a signed agreement before using a device.  Parents are also given the option to purchase insurance through the school at $25 per school year.  


 -Transportation Request Form
 – complete only if you will need transportation to/from school. A separate form must be completed for each student.


 -MHSAA Physical Form
 - complete for any student/athlete participating in FEAS athletics.  This completed form must be on file in the high school office prior to participation.


 -Health Appraisal Form
 – complete if enrolling in a licensed programs only (i.e. preschool, extended day care, summer adventure day camp)

 -Concussion Form - Required Parent/Student Concussion Information and Agreement Form


 -Online Free or Reduced Lunch Application
 – only one application is needed per family and can be completed online at www.lunchapp.com or obtained at any FEAS school office.
 

 

Directory Information Notice – By law, FEAS is required to release “directory information” on all students. To view a full definition of directory information, please refer to the FEAS student handbooks. If you have any objections regarding the release of this information about your child, please notify the school your child will be attending in writing.
